Leptospirosis Deaths Surpass Reported Cases in Poor Regions

In some of the Philippines’ poorest and most remote areas, leptospirosis deaths have exceeded the number of reported infections in recent years. This alarming trend suggests significant underreporting of cases, experts said. The unequal health systems in these regions likely contribute to this troubling disparity.

Data analysis by a University of the Philippines associate professor revealed that the Bangsamoro Autonomous Region in Muslim Mindanao (BARMM) and Soccsksargen have recorded more leptospirosis deaths than cases. This discrepancy points to many infections going undocumented, particularly in regions with limited healthcare access.

Case Fatality Ratios Reveal Underreporting

Dr. Rogelio Alicor Panao examined case fatality ratios (CFRs), which measure deaths relative to reported cases. “The BARMM and Soccsksargen regions have only a handful of recorded leptospirosis cases, yet these registered more deaths than infections,” he noted. This indicates that only severe cases and fatalities might be reported, while milder infections remain uncounted.

Other regions like Metro Manila, Central Luzon, and Western Visayas consistently report the highest leptospirosis cases nationwide. However, the CFRs in Eastern Visayas, Caraga, Soccsksargen, and BARMM have surpassed their reported case numbers in recent years, emphasizing a likely underestimation of infections.

Examples Highlight Data Distortions

In 2020, Soccsksargen reported a single leptospirosis case but recorded four deaths. Caraga had eight cases and 14 deaths the same year. BARMM logged only one case each in 2020 and 2021 but reported two deaths annually. “These distortions are not statistical quirks,” Dr. Panao explained. “They suggest underreporting where, possibly, only the most advanced cases and fatalities make it into official records, while many infections go undocumented.”

These findings align with data from the Philippine Statistics Authority (PSA), showing discrepancies in leptospirosis deaths and cases from 2012 to 2022. For instance, Metro Manila reported dozens of cases with a relatively low death rate, reflecting stronger health surveillance.

Impact of Unequal Health Systems

Despite Metro Manila’s large number of cases, its lower CFR indicates better healthcare reach and surveillance, officials observed. In contrast, the higher fatality ratio in under-resourced regions creates a misleading view of leptospirosis severity, making it seem deadlier where health systems are weaker.

“The incomplete reporting is possibly creating a false map of the disease: the leptospirosis situation in regions with better health systems appears milder, while under-resourced regions’ status looks deadlier than it really is,” experts explained.

Recent Leptospirosis Trends and Government Response

Following six storms this year, the Department of Health (DOH) reported 2,396 leptospirosis cases nationwide since June 8. This figure represents over half of the 4,575 cases recorded from January to September last year, indicating a significant rise.

However, the DOH has yet to release comprehensive yearly data on leptospirosis, only issuing partial updates when local governments report outbreaks. Recently, officials noted that the daily average of leptospirosis cases dropped sharply from nearly 200 per day to just 10 in the past week, signaling possible control measures taking effect.
